If you would like to purchase a sweet treat for your child, complete the form and send the completed form along with CASH to their teacher. THE DEADLINE to purchase for May is Friday, May 9th (NO EXCEPTIONS). NO REFUNDS will be issued after May 9th (including if your child miss school on the day the truck visits).
